If we are unable to bill insurance on your behalf, payment is due in full at the time of your appointment and we cannot guarantee 100% reimbursement from your insurance company as this is entirely dependent on your specific insurance plan. To assist you, Element will provide an itemized bill at the end of each session that can be submitted to your insurance company. We encourage you to call your insurance company ahead of your PT visit in order to better understand your benefits and how you can be reimbursed. We have created this form to assist you in the process.
